Students in your classroom could face a lot of troubles at home with abuse, not enough food, issues between family members, etc. When you find out one of your students is having trouble at home, do what you can to help them out but also modify or give extensions on the work their given so they can be successful. Some students may need to just stay in at lunch to complete their homework because it is not something they can do at home and give them the option to do that.
You may never find out what problems the students face individually at home but if you do, direct the students to the right supports. What are those? At school there's counselors, peer groups, mentors, psychologist, nurse, outside resources for help, breakfast club and academic resource room.
Can you think of any other supports that schools have that are there to help students when they are facing troubles at home?
